Online Services

GP online services allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:

  • book or cancel appointments online
  • renew or order repeat prescriptions online
  • view parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results

You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. More information including “how to” leaflets and videos of patients and why they are using GP online services are available at

How to sign-up

For more information, please contact reception or next time you visit the surgery, bring with you the completed application (link below), photo ID (passport, driving licence) and something with your address on like a bank statement or council tax bill but not a gas or electric bill. If you don’t have these, please talk to us and we will help you sign-up.  Application for Online access

Welcome to Glendale Surgery


If you are booked into the clinic, the weather forecast isn't too good at the moment but fingers crossed that will change so please:

Arrive at your appointment time and not before as you will have to wait outside.

Coming early will cause queues as appointment times are very specific at 1.5 minutes apart. Wait in your car if you need to.

Wear a short sleeved garment under your outer clothing and don't wear several layers. This will also help to keep any queues down.


If you have ANY symptoms of covid:

** A new continuous cough

** A temperature

** Loss or change to your sense of of smell or taste






Glendale Surgery is committed to providing high quality and readily available health care to the patients of this practice. We aim to offer continuity of care with the doctor of your choice as much as possible and see this as the key to developing a good doctor/patient relationship.

We are situated in the Cheviot Primary Care Centre, next to the free car park in Padgepool Place. The Centre houses another practice and the rest of the primary health care team. When you come in the main door, we are the surgery on the left hand side of the waiting area.

We are confident that our website will provide clear and concise help and give our patients the information they require in an easy and convenient format. It has been designed with the patient’s needs at the forefront of everything, from checking surgery times to letting us know what you think of us.

Serving You

Our dedicated team are here to treat those minor ailments that occur as well as providing specialist management of long-term conditions and clinics covering a wide range of healthcare issues. The technology also means you can now do a lot of things from the comfort of your home such as order a repeat prescription or cancel an appointment.
